Job summary

Sweetwater Care in California is seeking an LVN/LPN for ongoing nursing support in a long-term care setting. The role focuses on delivering care within scope of practice, following physician orders and center policies to meet residents' needs.

Qualifications include long-term care experience, a current LVN/LPN license, and a valid BLS/CPR certification. PointClickCare charting experience is preferred. Management experience is a plus but not required.

Qualifications

• Proficient with PointClickCare charting.
• Active LVN/LPN license in practicing state; BLS/CPR cert required.

Responsibilities

• Follow established charting procedures and incident reporting.
• Adhere to narcotic counting, pharmacy orders, and restraint reduction protocols.
• Document changes in patient conditions (nutrition, infections, skin issues, weight changes).
• Use Quality Life Manual to identify, place and monitor patients in programs.
• Notify departments of room changes, LOAs, new admits, or discharges.
• Occasionally rounds with physicians as needed.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
