Job summary

Sweetwater Care in Whitefish, MT is seeking an experienced Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to provide nursing care within the scope of practice, following physician orders, center policies and applicable guidelines.

You will document patient condition changes, participate in physician rounds as needed, and adhere to charting, narcotics, and safety procedures. Prior long-term care experience and reliable charting skills are essential.

Qualifications

• Active LVN/LPN license in the practicing state.
• Current BLS/CPR certification or willingness to acquire.
• Experience in long-term care settings.
• Experience in nursing management is preferred.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions, including nutrition, infections, skin issues, weight loss, or other changes.
• Uses the Quality Life Manual for patient identification, placement and monitoring.
• Documents and notifies departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
