Job summary

Sweetwater Care in Los Angeles, CA seeks an experienced Licensed Vocational Nurse (LVN) to oversee nursing operations in a long-term care facility. You will ensure care meets standards, follow physician orders, and comply with state regulations.

Responsibilities include coordinating care plans, medication administration, staff supervision, rounding with physicians, and educating patients and families. Prior nursing management is preferred but not required.

Qualifications

• California LVN license is required.
• 1 year nursing management experience preferred.
• Long-term care experience is required.
• Experience with PointClickCare charting is preferred.

• Utilizes the Infection Control Manual.
• Participates in the Quality Improvement Program.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Receives, transcribes, and carries out physician orders, if allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
