Job summary

Sweetwater Care is seeking a licensed LPN to provide nursing care within the scope of practice and in accordance with facility policies and state guidelines. The role includes leading nursing activities, coordinating care, and ensuring timely medication administration in a long-term care setting.

The ideal candidate has LPN licensure, prior nursing management experience preferred, and familiarity with Point Click Care charting.

Qualifications

• Licensed practical nurse (LPN) license in required state.
• One year of nursing management experience preferred.
• Experience in long-term care settings.
• Point Click Care charting experience or familiarity.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in and assists with Quality Improvement Program implementation.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies other departments of patient changes, including room changes, LOA, new admits, discharge.
• Provides family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders, as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.

Qualifications
