Job summary

Sweetwater Care is seeking an LVN with California licensure and nursing management experience to oversee resident care in a long-term care setting. The role requires coordinating patient placement, physician orders, and education for patients and families.

The ideal candidate will have experience with PointClickCare charting, a strong commitment to quality improvement, and a clear understanding of infection control policies within a facility environment.

Qualifications

• Licensed LVN in California; compact license if working in a compact state
• Minimum of 1 year in nursing management preferred
• Long-term care experience required or preferred
• Experience with PointClickCare charting and documentation

Responsibilities

• Provide nursing care within scope of practice and in accordance with standards and physician orders
• Participate in Quality Improvement and the Quality Life Manual programs
• Coordinate patient placement, changes in room, admits, discharges and notify departments as needed
• Educate patients and families about care plans and treatments
• Administer medications and treatments per facility policy and nurse practice act
• Receive, transcribe, and carry out physician orders as allowed by policy
• Round with physicians when necessary
• Ensure compliance with infection control policies and company guidelines

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
