Job summary

Sweetwater Care is seeking an LVN to join our long-term care team as a hands-on supervisor, ensuring high-quality patient care and adherence to facility policies.

The role emphasizes medication administration, treatment delivery, and active participation in quality improvement activities with close nurse-physician collaboration. CA licensure is mandatory, and prior LTC experience is valued.

Qualifications

• Must be licensed LVN in California.
• One-year nursing management experience preferred but not required.
• Experience in long-term care required.
• Proficient in PointClickCare charting and documentation.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Uses Quality Life Manual to identify and monitor patient programs.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely per facility policy.
• Administers treatments timely per facility policy.
• Transcribes and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient's needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.


• Understands, participates and assists in implementing the Quality Improvement Program.


• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.


• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.


• Conducts family and patient education.


• Demonstrates the ability to administer medications timely and according to facility policy.


• Demonstrates the ability to administer treatments timely and according to facility policy.


• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act


• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state CA,


• One-year prior nursing management experience preferred, not required.
