Job summary

Sweetwater Care in Pasadena, CA seeks an experienced LVN with management skills to oversee nursing services in our skilled nursing facility. You will ensure patient needs are met, coordinate with physicians, and supervise routine care.

The ideal candidate holds a California LVN license, has prior management experience, and familiarity with SNF/LTC settings. Bilingual candidates are a plus and may contribute to family education and quality initiatives.

Qualifications

• Licensed LVN in California is required.
• The role prefers at least 1 year of nursing management experience.
• Experience in skilled nursing facilities or long-term care settings is a plus.
• Bilingual abilities are beneficial.

Responsibilities

• Provides nursing care within scope of practice per policy and orders.
• Administers medications and treatments per facility policy.
• Makes rounds with physicians when needed.
• Communicates patient changes to departments and documents accordingly.
• Educates patients and families as required.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Bilingual is a plus.
