Job summary

Sweetwater Care in Huntington Park, CA is seeking an experienced LVN nurse manager to oversee nursing services in our long-term care center.

The role emphasizes infection control, quality improvement, patient and family education, timely medication administration, and adherence to physician orders and center policies.

Experience with Point Click Care charting is preferred to ensure accurate documentation and coordination with physicians.

Qualifications

• Currently licensed LVN in California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Utilizes Life Manual for patient placement and monitoring.
• Notifies Departments of patient changes.
• Conducts family and patient education.
• Administers medications timely per policy.
• Administers treatments timely per policy.
• Receives, transcribes, and carries out physician orders.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient's needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.
