Qualifications

• ASN or equivalent required.
• RN license for State or Compact required.
• 1 year home health nursing experience within last 3 years required.
• 1 year hospice nursing experience within last 3 years required.
• OASIS experience preferred.

Responsibilities

• Provide and coordinate care for hospice patients at home.
• Provide case management for assigned patient caseload.
• Perform Start of Care/Admission visits.
• Develop and implement nursing care plans including Start of Care and admissions.
• Support patient and family to stay at home and maintain quality life.
• Educate patients and families on disease management.
• Ensure proper transcription of physician orders and timely documentation.
• Notify physicians of significant changes in status.
• Provide bereavement support to families.

Pay Rate:Range $2040 to $2120 Weekly Gross pay (Non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)

MileageReimbursement: $0.49/mile for distancecovered between patient home appointments

Unit: Home Health Hospice

Shift: 8 Hours; Days, 8a - 5p

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri

Position Requirements:

• Registered Nurse: 1 year - Required
  
• Recent/Current HomeHealth Nursing experience: 1 year within the last 3 years - Required
  
• Recent/CurrentHospice Nursing experience: 1 year with the last 3 years - Required
  
• Home Care Home Base(HCHB) charting experience - Required

Job Description:

• Provide andcoordinate care for assigned hospice patients in comfort of their homes.
  
• Provide case management for assigned patient case load
  
• Perform Start of Care/Admission visits
  
• Develop and implementnursing care plans including Start of Care and admissions as assigned
  
• Support patientrecovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy.
  
• Empower patients andfamilies to reach healthcare goals by educating them on disease management.
  
• Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
  
• Ensure physicians'orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
  
• Ensure patients'status is assessed on an ongoing basis and pertinent information isgathered and documented.
  
• Notify physicians ofsignificant changes in status.
  
• Document and completecharting for all patient visits within 24 hours.
  
• Provide bereavementsupport to patient's families as needed.
