Sage GroupHealthCare Staffing is seeking an RN Hospice Case Manager for a 13-week travel/contract position in Portage, MI. Local and traveler candidates are welcome. Start date ASAP. Full-time travel/contract/ temporary role with 40 hours/week, Mon–Fri, no weekend on-call.

Includes a weekly gross pay of $2,000 with non-taxable travel/housing stipend and mileage reimbursement. Requires RN licensure, Home Health/Hospice experience, BLS, and a valid driver's license.

Formación

• RN with 1 year home health hospice experience required.
• Recent hospice nursing experience mandatory.
• Valid driver's license and auto insurance required.
• Covid vaccination or exemption required.
• HCHB charting and OASIS experience preferred.

Responsabilidades

• Provide and coordinate care for hospice patients at home.
• Develop and implement Start of Care and admission nursing plans.
• Document charting within 24 hours of visits.
• Notify physicians of significant changes in patient status.
• Provide bereavement support to families.
• Territories to cover: Portage, Kalamazoo and surrounding areas.

Position Requirements:

• Registered Nurse: 1 year - Required
• Recent/Current HomeHealth Nursing experience: 1 year – Required
• Recent/CurrentHospice Nursing experience: 1 year – Required
• Home Care Home Base(HCHB) charting - Highly preferred
• OASIS experience preferred
• Must have validDriver’s License and Personal Auto Insurance
• Must be CovidVaccinated or provide medical or religious exemption

Job Description:

• Provide andcoordinate care for assigned hospice patients in comfort of their homes.
• Develop and implementnursing care plans including Start of Care and admissions as assigned.
• Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
• Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
• Notify physicians ofsignificant changes in status.
• Document and completecharting for all patient visits within 24 hours.
• Provide bereavementsupport to patient families as needed.
• Territories to cover: Portage, Kalamazoo and surrounding areas
