Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse Case Manager for a 13-week travel/contract position in Denver, CO. Start ASAP; Full-time, Travel, Contract, Temporary.

Responsibilities include coordinating home health hospice care, developing care plans, and supporting patients and families to remain at home. 40 hours weekly, Monday–Friday with no weekend/on-call; travel stipend possible.

Qualifications

• RN with CO license and 1 year home health experience.
• Hospice nursing experience within last 3 years.
• OASIS experience highly preferred.

Responsibilities

• Provide and coordinate care at patients' homes.
• Develop and implement nursing care plans including Start of Care and admissions.
• Support patient recovery and ability to stay at home.
• Educate patients and families on disease management.
• Document visits within 24 hours and ensure orders are legible.
• Territories to cover: Aurora/Central Denver area.

Shift: 8 Hours; Days, 8a – 5p

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No weekend or on-call

Position Requirements:

• Registered Nurse: 1 year - Required
• Recent/Current HomeHealth Nursing experience: 1 year within the last 3 years – Required
• Recent/CurrentHospice Nursing experience: 1 year – Required
• Home Care Home Base(HCHB) charting - Required
• OASIS experience - Highly preferred
• Must have validDriver’s License and Personal Auto Insurance

Job Description:

• Provide andcoordinate care for assigned patients in comfort of their homes.

• Empower patients andfamilies to reach healthcare goals by educating them on disease management.
• Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
• Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
• Notify physicians ofsignificant changes in status.
• Document and completecharting for all patient visits within 24 hours.
• Provide bereavementsupport to patient’s families as needed.
