Responsibilities

• Offer case management and clinical oversight to interdisciplinary teams.
• Ensure orders are legible and transcribed per policy.
• Assess patient status continuously and document findings.
• Notify physicians of significant changes in patient status.
• Cover territories within 60 miles from Jefferson City office.

Pay Rate:Range $2000 to $2080 Weekly Gross pay based on a 40-hour work week (Includes non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)

MileageReimbursement: $0.49/mile for distancecovered between patient home appointments

Unit: Home Health

Shift timings: 8 Hours; Days, 8.00 am – 5.00 pm

Schedule: 40 hours/week guaranteed, five shifts per week, Mon-Fri | Expectation productivity points - 30 per week

Position Requirements:

• Registered Nurse: 2 years - Required

Job Description:

• Provide and coordinate care forassigned hospice patients in the comfort of their homes.
• Develop and implement nursingcare plans, including Start of Care and admissions as assigned.
• Complete patient documentation and charting for the assigned caseload before the next business day
• Case management and provideclinical oversight to interdisciplinary
• Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
• Ensure physicians’ orders arelegible if taken verbally and transcribed appropriately per policy andprocedure.
• Ensure patients’ status isassessed on an ongoing basis and pertinent information is gathered anddocumented.
• Notify physicians of significantchanges in status
• Document and complete chartingfor all patient visits within 24 hours.
