Qualifications

• Registered Nurse with 2 years – Required
• Hospice nursing in a Home Health setting: 1 year within the last 3 years – Required
• HCHB charting: 1 year – Required
• Must have valid Driver’s License and Personal Auto Insurance
• Must be Covid Vaccinated or provide medical or religious exemption
• ASN or an equivalent – Required
• BSN or equivalent – Highly preferred
• RN License for State of Indiana or Compact – Required
• BLS Certification from AHA or American Red Cross – Required

Responsibilities

• Provide and coordinate care for assigned hospice patients in the home.
• Develop and implement nursing care plans including Start of Care and admissions.
• Provide case management services for the assigned patient caseload.
• Support pain management and enable patients to stay at home with family.
• Educate patients and families to reach healthcare goals.
• Case management and clinical oversight to interdisciplinary teams.
• Ensure nursing practice complies with state regulations and protocols.
• Document and chart all patient visits within 24 hours.
• Territories to cover: Lewis and Lawrence counties

MileageReimbursement: $0.49/mile for distancecovered between patient home appointments

Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, with back up on call

Job Description:

• Develop and implement nursingcare plans including Start of Care and admissions as assigned.
  
• Provide case management services for the assigned patient case load.
• Support pain management andability to stay at home - enabling them to spend quality time with theirloved ones and doing the activities they enjoy
• Empower patients and families toreach healthcare goals by educating them on disease management
• Case management and provideclinical oversight to interdisciplinary teams
• Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
• Ensure physicians’ orders arelegible if taken verbally and transcribed appropriately per policy andprocedure.
• Ensure patients’ status isassessed on an ongoing basis and pertinent information is gathered anddocumented.
• Notify physicians of significantchanges in status
• Document and complete chartingfor all patient visits within 24 hours.
