Job summary

Helio Health, Inc. in Rochester, NY is seeking a Continuing Care Coordinator to provide rehabilitation services as part of the treatment team.

This full-time role based at the Rochester Inpatient Program offers a hourly pay range of $23.00 to $27.00 and includes a sign-on incentive up to $1,500. You will manage a caseload, conduct screening and intake, develop treatment plans, and coordinate referrals while maintaining confidentiality.

Qualifications

• Associate's Degree in human services or related field is required or equivalent experience
• One year counseling experience preferred
• Equivalent alternative combination of education and experience may be substituted for the educational requirements
• Knowledge of addiction/recovery, behavioral health, self-help group process and principles of counseling
• Proficiency with basic computer applications

Responsibilities

Overview

Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Continuing Care Coordinator will provide rehabilitation services to patients as a part of their overall treatment. The Continuing Care Coordinator will work on a Full-Time basis out of Helio Health’s Rochester Inpatient Program.

Pay Rate: $23.00 - $27.00 per hour.

In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!

Responsibilities

• Maintain a case load of patients seeking treatment from the effects of substance use and/or mental health disorders.
• Perform essential counseling responsibilities including screening, intake and orientation, assessment, evaluation and intervention, treatment planning, referrals, crisis intervention, counseling, patient education, case management, reporting and record keeping.
