Helio Health is seeking a Targeted Case Manager to support recovery from substance use and mental health disorders. This full-time role is based at the Insights of Helio Health CCBHC, with a flexible schedule within clinic hours and potential evening shifts.

Qualifikationen

Overview

Helio Health is looking for a Targeted Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is aFull-Time position and located at our Insights of Helio Health Certified Community Behavioral Health Clinic (CCBHC).

Schedule:Flexible schedule within the clinic’s operating hours Monday–Thursday, 8:00am–8:30pm and Friday - Sunday 8:00am - 4:30pm some evening hours required

In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!

Pay: $24.00 -$26.00 per hour.

Responsibilities

• Trains of staff, volunteers, and students/interns

  • Provides services according to agency policies, procedures, program guidelines, and governmental regulations.

  • Evaluates programs and identifies strengths/weaknesses.
  
  
  • Assists in problem resolution and implementation of supervisor approved program modifications.
  
  
  
  


• Participates in program administrative and clinical committees as assigned.


• Represents agency to the community.


• Other duties as assigned by supervisor.
