Job summary

Helio Health, Inc. is seeking a Targeted Case Manager to support recovery from substance use and mental health disorders in a full-time role located at Insights of Helio Health Certified Community Behavioral Health Clinic (CCBHC).

The position involves managing and coordinating services for patients in treatment programs, with a flexible schedule within clinic hours and some evenings. A sign-on incentive up to $1,500 is offered, plus comprehensive benefits and hourly pay of $24.00-$26.00.

Qualifications

• Associate's degree in a related human services field required; Bachelor's preferred.
• Two years of experience working with behavioral health and/or substance abuse issues.
• Valid NYS Driver's License preferred.

Responsibilities

• Maintains a caseload and performs case work: screening, intake, orientation, referrals, counseling, crisis intervention, discharge planning.
• Coordinates care with community providers and documents reporting/record-keeping; consults with professionals.
• Trains staff, volunteers, and students; performs quality assurance activities and program evaluations.
• Participates in committees and represents the agency to the community; performs other duties as assigned.

Overview

Helio Health is looking for a Targeted Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is aFull-Time position and located at our Insights of Helio Health Certified Community Behavioral Health Clinic (CCBHC).

Schedule:Flexible schedule within the clinic's operating hours Monday-Thursday, 8:00am-8:30pm and Friday - Sunday 8:00am - 4:30pm some evening hours required

In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!

Pay: $24.00 -$26.00 per hour.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including:
  • Screening, intake, orientation
  • Referrals
  • Counseling
  • Crisis intervention
  • Patient/resident education
  • Case management
  • Reporting and record keeping
  • Consultation with other professionals
  • Coordination of care with other community providers
  • Discharge planning
• Trains of staff, volunteers, and students/interns
• Performs Quality Assurance activities
  • Provides services according to agency policies, procedures, program guidelines, and governmental regulations.
  • Monitors provision of services when assigned.
  • Evaluates programs and identifies strengths/weaknesses.
  • Assists in problem resolution and implementation of supervisor approved program modifications.
• Participates in program administrative and clinical committees as assigned.
• Represents agency to the community.
• Other duties as assigned by supervisor.
