Qualifications

• Bachelor’s degree in any field required or 3 years HIV-related services with HS diploma/GED.
• Experience coordinating referrals and advocating for clients in HIV-related programs.
• Strong knowledge of community resources and social services aids.

Responsibilities

• Screen clients for Ryan White eligibility and maintain documentation.
• Assess social, economic, housing, food security, and transportation needs.
• Develop individualized support plans and address barriers to care.
• Provide referrals to housing, food, transportation, and utilities.
• Follow up on referrals to ensure service access and outcomes.
• Provide crisis intervention and advocacy with community agencies and programs.
• Educate clients on HIV transmission prevention and disease management basics.
• Assist with problem-solving around social determinants of health and medication adherence.
• Document assessments, plans, and interactions in the EHR per HIPAA standards.
• Build relationships with community organizations to connect clients with resources.
• Work independently with minimal supervision.
