Job summary

Desert Springs Post Acute in Desert Hot Springs, CA is seeking a dedicated Transportation Coordinator to organize and coordinate resident transportation for medical appointments and outings. You will liaise with residents, families, transportation vendors, and clinical staff to ensure timely, safe, and smooth service.

The role requires strong scheduling, communication, and problem-solving abilities, with a focus on resident care.

Qualifications

• High school diploma or equivalent required.
• Previous scheduling, transportation coordination, healthcare, or administrative experience preferred.
• Strong organizational and multitasking skills.
• Excellent verbal and written communication abilities.
• Proficiency with Microsoft Office and scheduling systems.
• Ability to work independently and manage competing priorities.
• Customer service-oriented with a professional and compassionate demeanor.
• Experience in a skilled nursing, post-acute, or healthcare setting is a plus.

Responsibilities

• Coordinate and schedule resident transportation for medical appointments, procedures, and other approved outings
• Communicate appointment details with residents, families, transportation providers, and clinical staff
• Track and maintain transportation schedules and appointment calendars
• Confirm appointments and transportation arrangements to ensure timely service
• Monitor transportation requests and prioritize urgent needs appropriately
• Maintain accurate records, logs, and documentation
• Coordinate with clinical and interdisciplinary teams regarding resident transportation needs
• Assist with administrative duties and special projects as assigned
• Ensure compliance with facility policies and resident confidentiality requirements
