Job summary

Desert Springs Post Acute in Desert Hot Springs, CA is seeking a Transportation Coordinator to coordinate resident transportation to medical appointments and other essential destinations. • High school diploma or equivalent is required.
• Previous scheduling, transportation coordination, or healthcare administration experience preferred.
• Strong organizational and multitasking skills.
• Excellent verbal and written communication abilities.
• Proficiency with Microsoft Office and scheduling systems.
• Ability to work independently and manage competing priorities.
• Customer service-oriented with a professional and compassionate demeanor.
• Experience in a skilled nursing, post-acute, or healthcare setting is a plus.

Responsibilities

• Coordinate and schedule resident transportation for medical appointments, procedures, and other approved outings.
• Communicate appointment details with residents, families, transportation providers, and clinical staff.
• Track and maintain transportation schedules and appointment calendars.
• Confirm appointments and transportation arrangements to ensure timely service.
• Monitor transportation requests and prioritize urgent needs appropriately.
• Maintain accurate records, logs, and documentation.
• Coordinate with clinical and interdisciplinary teams regarding resident transportation needs.
• Assist with administrative duties and special projects as assigned.
• Ensure compliance with facility policies and resident confidentiality requirements.
