Job summary

Upperline Health Plus is seeking a Primary Care Medical Assistant to deliver high-quality, patient-centered care in a value-based setting. The MA will room patients, collect histories and vitals, and support care coordination to improve outcomes and patient experience.

Qualifications

• Certified MA or minimum 2 years’ experience in a medical primary care setting.
• Experience in a primary care/outpatient clinic setting preferred.
• Certified MA or phlebotomy certification preferred.
• Knowledge of medical terminology, disease processes, and pharmacology basics.
• Ability to interpret provider documentation, orders, and treatment plans.
• Care coordination or care management experience ideal.
• Experience caring for adult/geriatric patients preferred.
• Wound care and foot care experience a plus.
• Work collaboratively in a team-based care model.
• EMR experience, AthenaHealth preferred.
• MS Office (Word, Excel) proficiency required.
• Reliable transportation and valid driver’s license.
• Strong customer-service orientation and work ethic.
• Ability to multitask in a fast-paced environment.
• Empathy, professionalism, and cultural sensitivity.
• Self-starter with problem solving skills.
• Excellent written and oral communication with patients and families.

Responsibilities

• Room patients, obtain and document vital signs, history, and complaints.
• Assist NP with health assessments during in-clinic visits.
• Support virtual provider encounters from clinic setting.
• Document clinical information in the EHR.
• Medication reconciliation and review of prescriptions and OTCs.
• Identify medication discrepancies and ensure reconciled lists are current.
• Telephonic outreach to coordinate preventive care.
• Handle in-basket messages, refills, and prior authorizations.
• Coordinate care: records requests, orders, scheduling, referrals.
• Identify community resources for patients and families.
