Job summary

Valley Health System is seeking a Patient Navigator for Luckow Pavilion on day shift. The role focuses on guiding patients through the health system, coordinating with departments to ensure timely access to services and collaborating with Nurse Navigator and care teams to address patient needs.

Ideal candidates will have an Associate degree (or higher) and Patient Navigator Certification; English fluency is required and PC proficiency is expected.

Qualifications

• Associates degree preferred; Patient Navigator Certification preferred or obtained within 1 year of employment.
• Basic understanding of medical terminology, health care systems and health care financing required.
• Proficiency with a personal computer is necessary.

Responsibilities

• Provide individualized service and navigate patients through the health system.
• Coordinate with departments to ensure timely access to services.
• Assist with diagnosis, treatment and supportive services planning.
• Collaborate with Nurse Navigator and health care professionals to address patient needs.
