The role supports RNs with basic patient care, monitors vital signs, and assists with feeding and grooming, ensuring comfort and safety. The PCT will maintain patient rooms, document care accurately and participate in hand-offs, while engaging with patients and families with compassion.

Qualifications

• Provides basic patient care and comfort
• Good communication with patients, families and staff
• Capable of adapting to a fast-changing healthcare environment

Responsabilités

• Provides basic patient care including vital signs, personal care and hygiene
• Acts as liaison between patient and nurse to report changes or concerns
• Delivers high-level customer service to patients, families, visitors and staff
• Documents patient care accurately and promptly
• Participates in hand-off of pertinent information on arrival and departure
• Performs delegated tasks once competency is validated
• Assists with safe patient transfers and mobility
• Encourages patient independence and individualizes care based on interests
