Hanford Post Acute is seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team in Hanford, CA. This full-time role offers per-diem daytime shifts and a competitive hourly rate. You will evaluate residents, develop treatment plans, and collaborate with an interdisciplinary team to support residents in regaining independence.

Required CA OT license and CPR certification. SNF/post-acute experience preferred; strong communication and organizational skills are essential.

Qualificações

• Must hold a California OT license.
• CPR certification required.
• Experience in skilled nursing or post-acute care preferred.
• Excellent communication and organizational skills.

Responsabilidades

• Evaluate residents and develop individualized treatment plans
• Deliver occupational therapy services in accordance with physician orders and care plans
• Document evaluations, daily treatment notes, and progress reports accurately and timely
• Supervise and support COTAs and therapy aides in accordance with state regulations
• Educate residents, caregivers, and staff on therapy techniques and post-discharge care
• Participate in care planning meetings and interdisciplinary collaboration
• Recommend adaptive equipment and assist with discharge planning
