Job summary

Hanford Post Acute in Hanford, CA is seeking an experienced Occupational Therapist (OT) to join our rehabilitation team on a per-diem schedule. The role requires a California OT license and CPR certification, with a focus on resident-centered care and interdisciplinary collaboration.

Responsibilities include evaluating residents, creating treatment plans, delivering therapy per physician orders, documenting progress, and supporting COTAs.

Qualifications

• Valid California Occupational Therapy License (required).
• Current CPR certification (required).
• Experience in skilled nursing or post-acute care preferred.
• Strong communication, clinical, and organizational skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver occupational therapy services in accordance with physician orders and care plans.
• Document evaluations, daily treatment notes, and progress reports accurately and timely.
• Supervise and support COTAs and therapy aides in accordance with state regulations.
• Educate residents, caregivers, and staff on therapy techniques and post-discharge care.
• Participate in care planning meetings and interdisciplinary collaboration.
• Recommend adaptive equipment and assist with discharge planning.
