Job summary

Hanford Post Acute in Hanford, CA is seeking a dedicated Occupational Therapist to join our rehabilitation team on a per-diem basis. You will assess residents, design individualized treatment plans, and deliver OT services in line with physician orders and care plans.

This role offers a supportive, team-oriented environment with opportunities to mentor COTAs and aides, participate in care planning, and help residents regain independence. California OT license and CPR are required.

Qualifications

• Valid California OT license required.
• Current CPR certification required.
• Skilled nursing or post-acute care experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver occupational therapy services in accordance with physician orders and care plans.
• Document evaluations, daily treatment notes, and progress reports accurately and timely.
• Supervise and support COTAs and therapy aides in accordance with state regulations.
• Educate residents, caregivers, and staff on therapy techniques and post-discharge care.
• Participate in care planning meetings and interdisciplinary collaboration.
• Recommend adaptive equipment and assist with discharge planning.
