Job summary

San Joaquin Nursing and Rehabilitation is seeking a Staff Occupational Therapist (OT) to evaluate and treat residents, collaborate with families and the interdisciplinary team, and maintain accurate medical records. The OT will also conduct in-services and training for facility staff on an ongoing basis.

The role requires CA OT licensure, and prior SNF or post-acute experience is preferred. PRN hours with competitive hourly pay are available, with opportunities for education and career

Qualifications

• Bachelor’s Degree in Occupational Therapy required; Master’s or Doctorate preferred.
• Experience in skilled nursing or post-acute setting preferred.
• New graduates welcome to apply.

Responsibilities

• Evaluate residents within 24 hours of referral and document promptly.
• Develop and adjust individualized treatment plans with physician approval.
• Provide skilled OT services per physician orders.
• Supervise COTAs, aides, and students per state acts.
• Communicate progress with the interdisciplinary team and families.
• Participate in care, rehabilitation, and review meetings.
• Document evaluations, daily notes, and recertifications per regulations.
• Assist with discharge planning and equipment recommendations.
• Educate families and staff on maintenance and safety.
