Job summary

Oakwood Gardens Care Center in Fresno, CA is seeking a licensed Occupational Therapist (OT) to join our skilled nursing and rehab team. This PRN position offers competitive hourly pay, full benefits, and an interdisciplinary environment focused on resident-centered care and meaningful outcomes.

Responsibilities include evaluating residents, developing treatment plans, providing direct therapy, supervising COTAs, documenting progress, and contributing to discharge planning and education.

Qualifications

• California-licensed OT required.
• Experience with SNF or post-acute rehab preferred but not required.
• Strong communication and collaboration with interdisciplinary team.

Responsibilities

• Evaluate residents to determine therapy needs and functional goals.
• Develop and implement individualized treatment plans.
• Provide direct OT services in accordance with physician orders.
• Supervise COTAs and collaborate with the care team.
• Document progress and outcomes for regulatory compliance.
• Assist in discharge planning and caregiver education.
• Participate in quality improvement initiatives.
