Job summary

San Joaquin Nursing and Rehabilitation is seeking an Occupational Therapist (OT) for a full-time role in a skilled nursing setting in California. The OT will evaluate and treat residents, collaborate with families and the interdisciplinary team, and ensure accurate documentation.

Qualifications

• Bachelor's Degree in Occupational Therapy required; Master's or Doctorate preferred.
• Experience in skilled nursing or post-acute setting preferred.

Responsibilities

• Evaluate residents within 24 hours of physician referral and document delays.
• Develop individualized treatment plans and obtain physician approval.
• Provide skilled occupational therapy services per physician orders.
• Supervise COTAs, therapy aides, and students per state practice acts.
• Communicate resident progress with the interdisciplinary team.
• Document evaluations and progress notes in compliance with regulations.
• Instruct families and staff on maintenance programs prior to discharge.
• Participate in discharge planning and recommend appropriate DME.
• Assist nursing staff with training of Restorative Aides.
• Report equipment issues to ensure safe operation.
• Maintain positive professional relationships with residents, families, and staff.
