Job summary

Orchard Post Acute is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team on a PRN basis in California. The OT will evaluate residents, develop individualized care plans, and help residents regain independence with a resident-centered approach.

Requirements include a CA OT license, graduation from an accredited OT program, CPR certification, and strong clinical and documentation skills. Prior skilled nursing or long-term care experience is preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current California Occupational Therapist license in good standing.
• Current CPR certification.
• Previous skilled nursing or long-term care experience preferred.
• Strong clinical, communication, and documentation skills.
• Compassionate, dependable, and resident-centered care.

Responsibilities

• Evaluate residents' functional abilities and develop individualized treatment plans.
• Provide occupational therapy services in accordance with physician orders and established plans of care.
• Help residents improve daily living skills, mobility, coordination, strength, and independence.
• Recommend and train residents in the use of adaptive equipment and techniques.
• Educate residents, families, and caregivers regarding treatment goals and safety strategies.
• Accurately document evaluations, treatments, progress, and changes in condition.
• Communicate resident progress and concerns to the interdisciplinary care team.
• Participate in care planning and discharge planning as needed.
• Follow all facility policies and applicable state and federal regulations.
