Job summary

San Joaquin Nursing and Rehabilitation is seeking an Occupational Therapist (OT) for a PRN position. The OT will evaluate and treat residents, collaborate with families, physicians and the interdisciplinary team, and maintain documentation in the medical record. In-services and staff training are also part of the role.

Qualifications

• Bachelor's Degree in Occupational Therapy required; Master's or Doctorate preferred.
• Licensed California OT in good standing.
• Experience in skilled nursing or post-acute setting preferred.
• New graduates welcome to apply.

Responsibilities

• Evaluate residents within 24 hours (or one business day) of physician referral; document clearly if delayed.
• Develop individualized treatment plans and obtain physician approval.
• Provide skilled occupational therapy services per physician orders.
• Supervise COTAs, therapy aides, and students in accordance with state practice acts.
• Communicate resident progress, concerns, and care plans with the interdisciplinary team.
• Participate in discharge planning and recommend appropriate durable medical equipment (DME).
• Document evaluations, daily treatment notes, progress notes, and recertifications in compliance with state regulations and payer requirements.
