Job summary

Advocate Health Care is seeking a patient care assistant to support daily activities for patients under the supervision of registered nurses. You will help with bathing, feeding, positioning, and transferring, while accurately documenting vitals and intake/output in the EMR.

The role includes assisting with admissions, transfers, and discharges, coordinating tests across departments, and maintaining infection prevention practices.

Responsibilities

• Assist RN with daily patient care including bathing, feeding and positioning.
• Support admission/transfer/discharge processes and room setup.
• Document vitals, intake/output and daily weights in EMR.
• Assist with rounds, patient transport and use of assistive devices.
• Support nutrition, skin care and proper use of TED hose and devices.
• Coordinate tests and treatments across departments and manage communications.
• Maintain infection prevention practices and proper PPE usage.
• Handle patient inquiries, phone calls and facility communications.
• Prepare chart forms for admissions and transfers; verify patient records.
