Job summary

Advocate Health in Sheboygan, WI is seeking a patient-focused Nursing Assistant to support daily patient care under RN supervision, including bathing, feeding, mobility assistance, and routine monitoring in a hospital setting.

You will document vitals and intake/output in the EMR, aid in admissions and transfers, restock supplies, keep areas clean, and follow infection control policies. BLS and Nurse Aide registration are required.

Qualifications

• Basic computer skills.

• Team members will maintain annual educational requirements for hospital based certifications (e.g. DNV, CARF or others).

Responsibilities

• Assist RN with daily patient care: bathing, oral hygiene, feeding and mobility.
• Measure and document vitals, I&O, daily weights, and other data in EMR.
• Perform purposeful rounding and communicate patient/family needs to RN.
• Assist with admissions, transfers and discharge planning; prepare room and belongings.
• Collect urine, stool and sputum samples and perform point-of-care testing as directed.
• Run errands, deliver items, and restock nursing area; clean equipment and areas.
• Assist with patient positioning and safe transfers using proper technique.
• Support skin care and positioning to protect skin integrity and use TED hose or similar devices.
• Follow infection prevention policies and proper PPE when in contact with patients.
• Demonstrate age-appropriate care and document observations for patient status.
• Follow site/unit requirements for nursing tasks and hand hygiene.
