Job summary

PACS is seeking a Certified Nursing Assistant to provide routine daily nursing care and services to assigned residents in accordance with care plans and supervision. The role emphasizes accuracy in documentation and timely reporting of changes in resident conditions.

Responsibilities include identifying residents, assisting with daily activities, maintaining room readiness, and supporting the nursing team to ensure patient safety and comfort.

Qualifications

• Must possess, as a minimum, an 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with laws of the state. CPR Preferred

Responsibilities

• Identify residents before administering treatments, serving meals, etc., using wristband or photo card.
• Record entries on flow sheets, notes, charts in an informative, descriptive manner.
• Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Assist residents with daily functions and personal care as directed by care plans.
• Keep residents’ rooms prepared and assist with moving, bathing, dressing, and positioning as needed.
• Observe and report skin integrity, injuries, or changes in condition to the nursing team.
