Responsibilities

• Provide direct resident care services including medicine distribution according to an individualized plan of care.
• Observe residents and report condition changes to physicians and supervisors with timely interventions.
• Document all observations, incidents, interventions, and daily medication care.
• Relay information to the next shift and ensure MAR documentation is up to date.
• Assist with activities of daily living and care planning to support resident independence.
