Job summary

Gravitas is seeking a Medication Aide to assist residents with medications under the direct supervision of the Director of Patient Health Services. The role includes preparing medications, supporting self-administration, and ensuring safety and accuracy in daily routines.

You will check expiration dates, review MARs, reorder medications, keep carts neat, and assist with activities of daily living while protecting resident privacy and dignity.

Qualifications

• Completion of Medication Assistance Training Program and competency by supervisor.
• Read MARs and follow resident medication instructions.

Responsibilities

• Assist with self-administration of medications as delegated.
• Check expiration dates on medications.
• Reorder medications through the resident’s chosen pharmacy.
• Read the communication logbook and note changes to supervisors.
• Review MAR to ensure changes are reflected and report updates.
• Keep medication carts neat and stocked.
• Provide medication reminders and assistance.
• Perform MAR review at the start of each month.
• Maintain accurate narcotics counts at shift start/end.
• Ensure refrigerator temperature is 36–40°F.
• Assist with activities of daily living as needed.
• Engage residents in meaningful activities.
• Report changes to Director of Patient Health Services or LPN Supervisor.
