Job summary

Gravitas is seeking a Medication Aide in New York, NY to assist residents with medications under supervision of the Director of Patient/Health Services. You will support daily living activities and ensure accurate medication handling.

Responsibilities include verifying expiration dates, reordering meds, reviewing MAR, maintaining narcotic counts, and ensuring proper refrigeration. You will document changes and communicate with supervising staff to maintain resident safety.

Qualifications

• Proficiency in assisting with medication administration and monitoring.
• Ability to support residents with activities of daily living.
• Experience documenting changes in medication and MAR records.

Responsibilities

• Assist with self-administration of medication as delegated by the Director of Patient/Health Services.
• Check expiration dates on medications.
• Reorder medications through resident’s chosen pharmacy.
• Read communication logbook and check with supervisor regarding changes in medication instructions.
• Review MAR to ensure changes are reflected and escalate as needed.
• Keep medication carts neat and clean and replenish supplies.
• Provide medication reminders and assistance.
• Perform MAR review at start of each month or as needed.
• Advise on updated prescriptions and re-order schedules.
• Maintain narcotics counts at start and end of shift.
• Check refrigerated medication temperature 36-40 degrees Fahrenheit.
• Assist with activities of daily living per individual service plans.
• Engage residents in meaningful activities to enhance quality of life.
