Job summary

OneWell Health Care is seeking a Field-Based Licensed Practical Nurse (LPN) to provide skilled nursing care to individuals with IDD in home and community settings across Pennsylvania.

You will work under the RN Case Manager, implementing care plans, administering medications, and supporting health, safety, and quality of life for participants. A Pennsylvania LPN license and CPR/First Aid are required.

Qualifications

• Current and active Pennsylvania LPN license.
• CPR and First Aid certification.
• Completion of an accredited Practical Nursing program approved by the PA State Board.
• Minimum of one year nursing experience.
• Experience with IDD, home health, or long-term care preferred.

Responsibilities

• Implement individualized care plans and administer medications.
• Perform wound care, catheter care, feeding tube management and vital signs monitoring.
• Educate families, caregivers and new staff; participate in team meetings and reviews.
• Maintain documentation in line with ISP requirements and nursing standards.
• Uphold infection control and safety practices; support quality improvement and audits.
