Job summary

OneWell Health Care in Pennsylvania is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services in home and community settings.

Under the supervision of an RN Case Manager, the LPN implements care plans, administers medications, performs wound care and catheter care, and supports individuals and families to maintain health and safety. This field-based role requires PA LPN license, CPR/First Aid, and a valid driver’s license with reliable transportation.

Qualifications

• Licensed Practical Nurse (LPN) license in Pennsylvania.
• CPR and First Aid certification are required.
• Minimum of one year of nursing experience; IDD/home health experience preferred.

Responsibilities

• Implements individualized care plans.
• Administers prescribed medications and treatments.
• Performs wound care and catheter care.
• Supports individuals and families to maintain health, safety, and independence.
