Job summary

Bethesda is recruiting a Licensed Practical Nurse for its Willmar, MN assisted living program. The LPN provides direct nursing care to home care clients, adheres to state regulations, and aligns with agency policies to support residents efficiently.

The role emphasizes client care, teamwork, and compliance. The LPN will work under RN supervision, with on-site duties in Bethesda’s assisted living settings and occasional travel to support client needs.

Qualifications

• Must be registered and licensed with the State of Minnesota as an LPN.
• At least 12 months’ experience in home care, long-term care, acute care or public health.
• Working knowledge of state requirements for the Comprehensive Home Care license and Minnesota Housing with Services registration (preferred).
• Ability to use computers, tablets and electronic records.
• Must be able to read, comprehend and follow instructions and work with accuracy.

Responsibilities

• Works under the direction of the RN to monitor clients’ needs and changes in condition and reports the results to the RN.
• Ensures that unlicensed staff members perform their duties and responsibilities in a timely, quality manner, and reports problems to RN.
• Reports changes in client condition or other concerns about clients to the RN in a timely manner.
• Initiates emergency procedures for clients and reports to RN.
• Reviews client care plans developed by the RN and consults with RN about client conditions and their treatments.
• Assists with implementation of physician orders.
