Job summary

Bethesda in Willmar, MN is seeking a Licensed Practical Nurse (LPN) for a full-time position in assisted living. The LPN will perform skilled nursing duties, administer medications, and ensure care plans are followed under RN supervision.

The role requires Minnesota LPN licensure and at least 12 months of home care or related experience; flexibility for shifts, on-call and potential travel within service area.

Qualifications

• Graduate of an accredited nursing program and Minnesota LPN license.
• Minimum 12 months’ experience in home care, long-term care, acute care, or public health.
• Knowledge of Minnesota requirements for home care licensing and services.

Responsibilities

• Monitor clients’ needs and changes in condition under RN supervision.
• Ensure unlicensed staff perform duties timely and with quality; report issues to RN.
• Administer medications and treatments; transcribe physician orders accurately.
• Participate in charting, documentation, and care plan reviews with the RN.
• Assist with onboarding and mentoring new staff; support client transitions.
