Qualifications

• Licensed Practical Nurse in the state you will be working as.
• IV Certification required or willing to obtain within six months of employment.
• CPR/AED Certification required or willing to obtain within six months of employment.
• Ability to assess geriatric nursing care needs.

Responsibilities

• Provides basic nursing care by assisting in resident assessments on admission and ongoing basis.
• Documents assessments accurately; communicates significant changes to IDT and residents’ families.
• Participates in care planning and updates the plan as interventions change.
• Delivers nursing interventions including medications and treatments safely and timely.
• Notifies physician promptly using SBARR; maintains composure in crises.
• Monitors care plan delivery and addresses issues via QAPI processes.
