Job summary

Trilogy Health Services, LLC is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants for residents on the unit. The LPN will observe residents daily, assess changes, and coordinate with the charge nurse and physician as needed.

The position emphasizes holistic care within the LPN scope, medication monitoring, and completion of required medical documentation.

Qualifications

• Must have and maintain a current, valid state LPN license.
• CPR certification required and current.

Responsibilities

• Visit residents daily to assess status and alert charge nurse/physician if conditions change.
• Identify problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Provide holistic nursing care within the LPN scope.
• Monitor medications, supplies, and equipment availability.
• Complete medical forms, reports, evaluations, and charting as needed.
• Report occupational exposures per facility policies.
