Job summary

Trilogy Health Services, LLC is seeking a Licensed Practical Nurse (LPN) to provide direct, quality clinical care and supervise nursing assistants for residents on their unit. The role emphasizes holistic patient care and timely communication with nursing leadership.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents daily on assigned unit to observe status and notify the charge nurse, physician, or Director of Nursing when a change occurs.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the LPN scope.
• Assist in monitoring medications, supplies, and equipment to ensure adequate stock.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting as needed.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policy.
