Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) to provide direct, high‑quality clinical care and supervise nursing assistants for residents on the unit. You will visit residents daily, assess health changes, respond to emergencies, assist with medications and supplies, and complete charting while upholding facility policies.

Qualifications

Must hold an active LPN license in the state.
CPR certification required.
Experience supervising nursing assistants preferred.

Responsibilities

Visit residents on assigned unit daily to observe and evaluate each resident’s health status and notify the charge nurse or physician of changes.
Identify resident problems and emergency situations and initiate lifesaving measures in the absence of a physician.
Provide holistic care within the LPN scope.
Monitor medications, supplies, and equipment to ensure availability for residents.
Perform administrative duties such as charting and forms as necessary.
Report occupational exposures per facility policies.
