Job summary

Trilogy Health Services is seeking a licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants on the unit. The role emphasizes compassionate, quality care for residents and collaboration with the nursing team.

Responsibilities include daily resident assessments, emergency response, medication monitoring, and administrative duties. A valid LPN license and CPR certification are required to join our Thrive benefits program and supportive work environment.

Qualifications

• Licensed Practical Nurse (LPN) license is required and must be maintained.
• Current CPR certification is required.

Responsibilities

• Visit residents on assigned unit daily to assess status and notify nurse or physician of changes.
• Identify problems and emergencies and initiate lifesaving measures in absence of physician.
• Provide holistic care within LPN scope of practice.
• Monitor medications, supplies, and equipment to ensure availability for residents.
• Perform administrative duties like charting and reporting as needed.
• Report exposures to blood, body fluids, infectious materials, and hazardous chemicals per policy.
