Job summary

Trilogy Health Services, LLC is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants for residents on their unit. You will visit residents daily to assess health status and coordinate with charge nurses, physicians, and leadership regarding changes in condition.

The role requires an active LPN license and CPR certification, with responsibility for medication monitoring, documentation, and adherence to facility policies.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents daily to observe and evaluate health status and notify the charge nurse or physician when changes occur.
• Identify problems and emergency situations and initiate lifesaving measures in absence of a physician.
• Provide holistic care within the LPN scope.
• Monitor medications, supplies, and equipment inventory to ensure availability.
• Complete medical forms, reports, evaluations, and charting as needed.
• Report exposures to blood and infectious materials per facility policy.
