Job summary

Trilogy Health Services in Ohio is seeking a Licensed Practical Nurse (LPN) to provide direct, high-quality clinical care and supervise nursing assistants on the unit. Visit residents daily, monitor condition changes, document care, and support the care team.

Qualifications

• Must have active LPN license in state of Ohio
• Current CPR certification required
• Experience in long-term care or skilled nursing preferred

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s status and notify the charge nurse or physician if changes occur
• Identify resident problems and emergencies and initiate lifesaving measures if needed
• Provide holistic care within LPN scope
• Assist in monitoring medications, supplies, and equipment to ensure adequate stock
• Perform administrative tasks such as charting and forms
• Report occupational exposures per facility policies
