Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) to provide direct, quality clinical care and supervise nursing assistants on the unit. You will monitor residents, report changes to the nursing leadership, and ensure holistic care within your LPN scope.

Responsibilities include daily resident visits, assessing physical and emotional status, and taking immediate action in emergencies when a physician is not available.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
