Qualifications

• Licensed Practical Nurse with state license.
• Excellent communication with residents, families and staff.
• Strong time management and organizational skills.

Responsibilities

• Provide basic nursing care and assess residents on admission and ongoing.
• Document assessments accurately and inform IDT of changes.
• Participate in care planning and revise the plan as needed.
• Administer medications and treatments safely; IV only if certified.
• Notify physician using SBARR and respond calmly to crises.
• Assist with ADLs and respond to call lights.
