Job summary

UF Health in Saint Augustine, FL is seeking a Licensed Practical Nurse to provide direct patient care under RN supervision. The LPN will administer medications, assist with IV therapy, perform phlebotomy, wound care, and routine assessments as part of the treatment team.

Responsibilities include documenting vitals, monitoring patient status, and collaborating with PCTs and the interdisciplinary team to ensure high-quality patient care.

Responsibilities

• Provide direct nursing care and implement individualized plans of care for assigned patients.
• Observe and assess patient behavior, symptoms, and responses to treatments and interventions.
• Monitor and document vital signs, reporting findings to the RN.
• Recognize signs of patient deterioration and collaborate with the RN to adjust monitoring or transfer as needed.
• Oversee and participate in patient care activities performed by Patient Care Technicians, including obtaining vitals, assisting with bathing and hygiene, linen changes, monitoring dietary intake, and aiding ambulation.
• Collaborate with the interdisciplinary healthcare team to ensure quality patient care.
• Perform phlebotomy procedures as ordered, including venipuncture method selection, collecting specimens, labeling, and monitoring for adverse reactions.
• Conduct bedside glucose monitoring as appropriate.
• Implement preventive skin care and wound care per physician orders; assess healing and report changes to the RN or supervising clinician.
• Provide patient and family education on health conditions, medications, wellness, follow-up care, and preventive practices.
• Maintain accurate and timely documentation per organizational policies while performing other duties as assigned.
